As a manager myself I want to add the prespective that your manager may need to hand in documentation for an out-of-cycle promotion/raise and that is the reason behind asking for the information. One would assume the manager should know these things and be able to document themselves but I find that this type of documentation is usually asked of for the employee to write since you have first hand supporting facts.
